Geriatric Medicine Position – Skilled Nursing, Saratoga Springs, New York
Compensation Range: $220K - $250K
Seeking a Family Medicine or Internal Medicine physician with Geriatric Medicine training or interest for clinical role in highly regarded Long Term Care facility in Saratoga Springs, New York
Join Saratoga Hospital Medical Group – Wesley Providers. As an employee of Saratoga Hospital Medical Group, you will work at Wesley Health Care Center, with residents in Long Term Care and Rehabilitation programs.
Position Overview:
Responsibilities include admissions to the skilled nursing facility and rehab units, readmissions after hospitalizations and quarterly evaluations of residents. Monthly evaluations and acute sick visits are seen with the assistance of two nurse practitioners.
Collaborate with other members of the Geriatric team including advanced practice providers, case management, social work and other interdisciplinary care teams.
This is an 80-hour biweekly position, Monday through Friday. Call will be inclusive of one weekend per month and shared call for evening hours from end of the day until 8 p.m. weekly, when night call takes over.

The Wesley Health Care Center provides compassionate skilled nursing care for seniors and chronically ill adults, as well as memory support and hospice care services in Saratoga Springs. Our long-term care residence spans three buildings, which include private and semi-private resident rooms, dining and social areas, a therapeutic pool, and therapy and rehabilitation rooms.

Long-Term Care Services for Seniors & Chronically Ill Adults
- Skilled Nursing Care
- We provide around-the-clock skilled nursing care in a residential community for aging adults who have significant, chronic medical needs.
- Light Care Geriatrics
- For older adults who require daily support, but whose needs are not as medically intense, we provide specialized long-term care, separate from our skilled nursing patients.
- Care for Chronically Ill Adults
- We provide specialized care for adults of any age who have suffered an illness or injury resulting in the need for chronic care and assistance.
- Memory Support
- We have a special unit designed to care for individuals dealing with the early stages of Alzheimer’s disease and chronic dementia. Our proactive programs and activities are designed to keep residents stimulated and alert in all aspects of life
Requirements for this opportunity include:
- Board Certification in Internal Medicine or Family Medicine
- Long Term Care Experience preferred
- Geriatric Medicine or Hospice & Palliative Care Certification preferred, not required
