Cases and Procedures

• No procedures required
• Common conditions: diabetes, CHF, hypertension, mild cognitive impairment
• Traditional primary care for older adults

Requirements

• MD or DO, Board Certified in Internal or Family Medicine
• Active Illinois license and DEA
