Company Profile

Community Connections, LLC

Company Overview

Community Connections, recognizes that each individual has a unique story, and his or her own distinctive strengths and challenges. With input from the individual, his or her loved ones, and our Eating Disorder experts, we develop an individualized eating disorder treatment plan for each member.

Our team help each individual develop the skills he or she will need to experience recovery. Residential treatment and the Intensive Outpatient program provide individuals with a supportive peer group that understands the challenges that arise from an eating disorder. By living together or participating in the Intensive Outpatient clinic with continuous therapeutic support, individuals learn to practice making healthy decisions when they experience feelings that accompany their eating disorder. Individuals learn to be mindful of feelings like anxiety, fear or shame, and each individual learns how to experience those feelings in a way that won't compromise their life or their recovery.

Company History

Community Connections, LLC was founded in 2010 in Phoenix, Arizona, with the specific mission of serving community members struggling with Eating Disorders or Disordered Eating alongside co-occurring mental illnesses (often referred to as dual diagnosis).
The company operates as a non-profit, but is a for-profit behavioral health agency.

The Development of a Specialized Program

The agency was co-founded by Liz Rodgers, MSW, who served as the Director and was instrumental in the development and inception of Community Connections. Driven by a passion for social work and a commitment to serving individuals with limited treatment opportunities, Ms. Rodgers focused the agency's expertise on providing high-quality, specialized care for eating disorders, rather than offering many services broadly.
The agency's programs were designed with a commitment to evidence-based practice and a holistic, person-centered approach.

Growth and Program Offerings

Community Connections, LLC established itself as a multi-level care provider, offering treatment options essential for full recovery from eating disorders:
Intensive Residential Treatment Program: Providing a home-like setting where individuals receive an initial evaluation from a multidisciplinary team (Primary Therapist, Registered Dietitian/Nutritionist, and Registered Nurse), followed by an individualized treatment plan.
Intensive Outpatient Program (IOP): Offering continuous therapeutic support, allowing individuals to practice making healthy decisions while engaging with a supportive peer group.
Key to their program is the integration of diverse, evidence-based therapies, including: CBT, DBT, Emotion-Focused Therapy, MI, and EMDR.

Treatment Philosophy and Impact

From its founding, the agency adopted a philosophy of practical, "real-world" recovery.
This is evident in its use of experiential treatment such as:
Meal Planning and Preparation
Food Challenges
Grocery Store Tours
Mindful Movement/Yoga
This practical approach helps clients bridge the gap between structured treatment and daily life. The agency's dedication to its clients is reflected in the numerous strong testimonials from individuals who credit Community Connections with saving their lives and providing the tools necessary for long-term recovery.

Community and Partnerships

Operating in Phoenix, Arizona, Community Connections has worked with various insurance providers, including Banner University Health Plans, Health Choice Arizona, Arizona Complete Health, and Mercy Care Plan, ensuring access to care for a wide segment of the community.
The history of Community Connections, LLC is defined by its specialization in complex dual diagnosis cases, its comprehensive and practical approach to eating disorder recovery, and its profound, life-changing impact on the individuals it serves.
