Job Summary:

• Assess patients' appropriateness for home care, learning needs, and progress toward goals.
• Collaborate with patients, families, and physicians to develop and implement care plans.
• Perform baseline assessments of swallowing, communication, and cognitive abilities.
• Maintain a safe environment for patients and co-workers, adhering to infection control measures.
• Administer therapy services per physician orders, including communication, swallowing, cognitive training, and therapeutic exercises.
• Provide discharge planning and referrals as needed.
• Communicate with interdisciplinary teams to ensure coordinated care.
• Document patient education and revise teaching plans as necessary.
• Ensure compliance with agency policies, home care regulations, and insurance guidelines.
• Participate in team meetings and maintain visit productivity standards.
• Travel to patient homes and facilities using a personal vehicle.
• Assist with agency growth through orientation of new employees and cross-training.

Required Qualifications:

• Minimum of one (1) year of experience in acute, subacute, or home care settings.
• Graduate of an accredited speech and language program with a Master's degree.
• Current NJ Speech Therapist (ST) License.
• Strong customer service and communication skills.
